As a patient, we encourage you verify that the physician you want to see is included in your insurance plan. Some plans have geographical restrictions.

Payment Options:
As a courtesy, we will submit medical claims to your primary and secondary insurance provider.
We are always here to assist you, but we rely on you to provide us with the most current insurance information.
You will not receive a bill from
Family Medical Center until your insurance provider has processed your claim. If your insurance does not cover the full amount of the bill, you will receive a
bill for the remainder of the charge. If you believe your insurance provider handled your claim incorrectly, please contact your insurance provider directly.
It is your responsibility to follow up with your insurance carrier if they are overdue on paying your charges.
You are ultimately responsible for
your account. It is your responsibility to make sure we have the correct billing information on file for you and your family members. Please notify us at 320-632-6611
if there are changes to your insurance plan, your name or address.
Copayments:
Many insurance companies and managed
care plans have copayments (copays). Usually, your insurance card will indicate if a copay is required. If your card does not show whether you owe a copay,
please call your insurance provider.
Like most clinics, we request that you pay your copay at the time of your appointment. Please give your copay to
the receptionist when you check in to see your doctor.
Referrals:
All referrals must be discussed with your
physician at Family Medical Center and arranged PRIOR to your consultation with the specialist. Retroactive or unauthorized referrals will not be approved.
